2026 Hilliard Bradley Baseball Summer Youth Camp Sign-Up

ACKNOWLEDGMENT OF RISK AND LIABILITY WAIVER I understand that participation in baseball activities, including drills, practices, games, and related camp activities, involves inherent risks of injury, including but not limited to falls, collisions, being struck by equipment or baseballs, and other risks associated with athletic participation. By registering my child for this camp, I acknowledge and voluntarily assume these risks on behalf of my child. I certify that my child is physically able to participate in camp activities. To the fullest extent permitted by law, I release and hold harmless the Hilliard Bradley Dugout Club, its officers, directors, coaches, volunteers, camp staff, and affiliated organizations from claims arising out of ordinary negligence related to my child's participation in the camp. In the event of a medical emergency, I authorize camp personnel to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses. This camp is organized and operated by the Hilliard Bradley Dugout Club and is not sponsored, endorsed, administered, or operated by the Hilliard City School District. The opinions, activities, products, and services associated with this camp are solely those of the Hilliard Bradley Dugout Club and do not necessarily reflect the views of the Hilliard City School District. *
